The leadership shift that changes everything

There is a moment that almost every independent dentist experiences, although few of us ever talk out loud about.

Our team’s gone home. The lights are still on. Charts are open. Insurance claims still need attention. Payroll is due, and of course, there’s a difficult conversation with an employee waiting in the wings we really wish would magically go away.

The day’s production was respectable, but collections were soft. There are two hygiene openings tomorrow still wide open, and our associate is asking for more autonomy and more money.

You know that moment, when you slouch a little in your chair and wonder, after all these years of debt, CE, and hard work:

Why is this still so hard?

I mean, you built a successful practice. Your patients trust you. Your clinical outcomes are solid. Your reputation is strong, and from the outside looking in, you’ve made it.

Yet somehow, this whole success thing feels a lot heavier than you expected. Not because the dentistry is difficult. But because leading the business of dentistry is. No one prepared you and I for that.

Dental school taught us how to diagnose disease, restore teeth, and improve lives. Continuing education sharpened our clinical skills. But somewhere along the way, a quiet assumption took hold.

If I just become an excellent dentist, I’ll naturally build an excellent practice.

It’s an understandable belief. I know I used to believe it. But if I’m honest, it left me feeling exhausted, and very much like I was starring in Groundhog Day rather than Bill Murray.

My practice improved. My life didn’t. Maybe you feel like I used to: you own a successful practice that somehow owns you.

Your frustration isn’t so much the working hard, we’re good at that. It’s more that despite working harder, the same problems keep circling back.

  • The same hiring challenges.
  • The same accountability issues.
  • The same communication breakdowns.
  • The same stress.
  • The same feeling that every improvement is temporary before another fire starts.

If that sounds remotely familiar, you’re not alone. More importantly, if your practice is like mine or the 150+ we coach, the problem isn’t at all what you think it is.

 

The Wrong Question

Most dentists ask questions like:

  • How do I find better employees?
  • How do I get my team to care more?
  • How do I increase production?
  • How do I become more efficient?

They’re reasonable questions. They’re just not the first questions. Because they focus on outcomes instead of causes.

Imagine standing beneath an apple tree that’s producing bitter fruit. You could polish every apple. You could spray the leaves. Water the branches. You could even hang prettier fruit on the outside.

Reality is, none of it changes the tree. Healthy fruit begins below ground, with healthy soil.

Guys, our practices are no different. What we experience every day, our culture, our teams, our patient experience, our financial results, that’s the visible fruit.

The healthy soil that makes it happen is the leadership we bring.

Don’t take this the wrong way, because I had to own it too: the practice you’re experiencing today is the result of leadership decisions made yesterday.

Here’s the win: it means we can learn to lead, and totally change our trajectory.

 

The Build Different Shift

My team and I believe exceptional practices aren’t built by accident. They’re designed. And every intentional practice begins with one framework that changes everything:

Identity → Beliefs → Decisions → Behaviors

It’s profound. It’s work. It’s also pretty simple. Let’s walk through it.

Identity Answers One Question

Who do I believe I am?

Most dentists understandably identify as a dentist, a clinician, maybe a business owner. Those identities aren’t wrong. They’re just incomplete.

If your identity stops at clinician, your decisions will almost always prioritize clinical excellence first, admirable, sure, but your practice requires more.

What if your identity shifted to this? I am a leader who happens to practice dentistry.

Now leadership is no longer something you do after patients leave. It becomes your work, and clinical dentistry is one expression of it.

Beliefs Are Shaped by Your Identity

If I believe I’m primarily a clinician, I’ll naturally believe things like:

  • The best dentistry wins.
  • Patients stay because of my clinical skill.
  • If I work harder, the practice will improve.

Again, not entirely false, just incomplete. Instead, imagine the beliefs of someone who identifies as a leader.

  • People determine the quality of my practice.
  • Character-driven culture influences every patient experience.
  • Systems create consistency.
  • Leadership multiplies clinical excellence.

Same practice. Entirely different beliefs, which means:

Decisions Look Different Because Beliefs Are Different

One dentist hires quickly because they’re desperate. Another hires slowly because they’re protecting a character-driven culture.

One avoids crucial conversations, hoping things get better on their own. Another views those conversations as investments in people.

One spends money on another piece of technology. Another invests first in leadership development, so that tech delivers what it was intended to.

Neither dentist is intentionally making bad decisions. They’re simply making decisions consistent with what they believe. And remember where those beliefs are rooted: your identity.

Behaviors Become Daily Behaviors

How you begin morning huddles. How often you coach your team. How consistently you hold people accountable. How clearly you communicate expectations. How frequently you recognize great work. How confidently you make difficult decisions.

These behaviors, over time, become your culture. Patients notice them. Your team experiences them. Your business reflects them. Which brings us to an uncomfortable but very liberating realization:

Your culture is not what you hope for. Your culture is what your leadership repeatedly creates.

 

Why Great Dentists Build Average Practices

Here’s the painful truth. Many average practices are led by extraordinary dentists.

  • Not because they lack intelligence.
  • Not because they lack effort.
  • Not because they lack passion.

Because no one ever taught them to intentionally develop the identity of a leader.

As a profession, we’ve unintentionally asked clinicians to build organizations. Those are different disciplines.

Clinical excellence earns trust. Leadership multiplies impact.

One improves individual patients. The other improves every patient who walks through your doors, because it shapes the people, process, and culture surrounding the clinical experience.

A Practice Is Always Becoming

Think about the oldest building in your community. It didn’t become beautiful overnight. It was designed. Every hallway. Every doorway. Every window. Someone decided what it would become long before it existed.

Your practice deserves the same intentionality. Yet many aren’t designed at all: they just evolve.

  • Processes accumulate.
  • Habits develop.
  • Expectations drift.
  • Culture forms by default.
  • Months become years.
  • Years become decades.

Then one day we wake up and wonder:

How did I get here?

Typically because of a thousand tiny leadership decisions along the way.

 

Reflection

Before moving on, I hope you’ll spend a few quiet minutes considering these questions. I promise you, they will help you.

  1. When you introduce yourself, do you think of yourself first as a dentist or as a leader?
  2. What belief about practice ownership have you accepted without ever questioning?
  3. Which recurring challenge in your practice might actually be rooted in leadership rather than operations?
  4. If your team’s experience reflects your leadership, what is it saying today?
  5. What kind of leader does the practice you’re trying to build actually require you to become?

 

Now: Take 1 Leadership Action This Week

Set aside thirty minutes this week. Create a document, or go old school and take out a blank sheet of paper.

At the top, write:

Who must I become for my practice to become what I envision?

Not: What should I do?
Not: What system do I need?
Not: What technology should I buy?

Start with identity.

Write five statements beginning with:

I am the kind of leader who…

Examples to borrow:

  • I am the kind of leader who has difficult conversations with respect and clarity.
  • I am the kind of leader who develops people instead of rescuing them.
  • I am the kind of leader who designs culture intentionally.
  • I am the kind of leader who builds systems that create freedom.

Then read those statements every morning for the next thirty days.

 

Build Different

There is a common belief in dentistry that building a better practice requires working harder. I’m here to tell you I’ve never found that to be true. Better practices begin with better leadership.

When you think differently, you make different decisions. When you make different decisions, you create different behaviors. When you consistently live different behaviors, you build a different culture.

And different cultures produce extraordinary teams, remarkable patient experiences, financial freedom, and a life with greater purpose.

Exceptional practices are not accidents. They are intentionally designed.

Our story with IgniteDDS began almost five years ago when my wife and I purchased an established fee-for-service practice. At the time, we promised the team that everything would stay the same. The practice was thriving, and from the outside it seemed clear they had found a successful recipe for success. Our goal wasn't to change it—we simply didn't want to mess it up.

What we discovered was that while everyone wanted the outcome of success, there wasn't alignment around how we achieved it or measured it. Success in room 1 wasn't the same as Room 2. It took us nearly four years to fully understand that because we were doing well. During that time, we also realized we weren't leading as effectively as we could have been. We weren't communicating a clear vision, establishing consistent standards, or implementing clear protocols. Instead, we allowed people to continue doing things their own way. Some team members appreciated that freedom, while others felt frustrated by the lack of consistency and direction.

After four years, we found ourselves at a crossroads. We felt stuck. Morale was low and we weren't sure we could get the practice where we wanted it to go on our own.

As owners who value relationships as much as results, David immediately stood out to us. His personality was authentic, approachable, and trustworthy. He was someone we could see ourselves learning from and building a genuine relationship with. Being local was a bonus, but what really resonated with us was that his vision for practice growth, demonstrated through his own private practice, closely mirrored our own. Today, our practice is more aligned, more accountable, and better equipped to deliver the experience we've always envisioned. Our collections have improved significantly, particularly through implementing systems that helped us address and recover outstanding balances.
