The Start-Up Ownership Illusion: Part 2

Margins”. Why owning a practice no longer guarantees wealth, and what actually does.

The Math Doesn’t Add Up Anymore

There was a time when owning a dental practice was simple:

  • Work hard → produce more → make more

I’m going to share a hard truth. That model is gone, and most future owners don’t realize it yet.

Here’s the Truth

Costs are rising:

  • Labor
  • Supplies
  • Technology
  • Rent

Reimbursements? Flat or declining.

So if your plan is:

“I’ll just work harder and produce more…”

You’re walking into a system that’s already working against you.

What Isn’t True

Most associates think ownership equals financial freedom. Without best practices, what they often build is:

  • Higher stress
  • Thinner margins
  • More complexity
  • Less take-home income than expected

Not because ownership is broken. Because their model is outdated.

The Separation Point

The top 2% don’t rely on production alone. They build practices around:

  • Treatment acceptance systems
  • Complete care philosophy
  • Schedule optimization, not just volume
  • Team-driven production, not doctor-dependent

Also known as, they don’t chase dentistry. They attract and design for it.

Ask Me How I Know

I’ve seen incredibly talented clinicians struggle financially, and I’ve seen average clinicians build highly profitable practices.

The difference, and you know this, is not clinical skill. It is a system design.

The Core Reframe

If your future practice depends on you doing more dentistry to succeed, you’ve already capped your growth.

Real margin comes from:

  • Better decisions
  • Better communication
  • Better systems

Not just better hands.

The Tactical Shift

Before you ever open:

  • Decide what percentage of your dentistry will be complete versus tooth at a time
  • Map your ideal day, not just a full day
  • Understand how treatment acceptance is greater than production volume

Because the practices winning today aren’t busier. They are smarter.

Wrapping Up

Ownership still works extremely well. But only if you build for today’s reality, not yesterday’s nostalgia.

If you’re paying attention, you’re already ahead of most. If you’re not, you’ll feel it in your margins first.

If you’re serious about ownership, learn how the best practices actually operate. That’s the difference between living to work and working to live.
